The villainess Fury (daughter of Wonder Woman on Earth-2) was previously imprisoned on an island. This story opens with her attempting to escape from the prison. From the dialogue, it's explained that she feigned sickness and was taken to the prison medical bay, where she overcame (and possibly killed) a female security guard named Molly Hutchins and stole her uniform.
